Medium no longer works. It crashes at startup

I know there are many open forums on this subject but i have yet to get a solution to this problem.  I have used Medium only a handful of times but now it won't even start.  It crashes immediately.  I have attempted different suggestions with no success.  I have uninstalled and reinstalled.  I have uninstalled and disabled EVGA OCX.  I am attaching my log file from my Medium folder to see if this will help with getting assistance.  I'm not sure how to contact any support for Medium for their assistance so if anyone knows please let me know.  I'm also attaching an image of the error that I get.  PLEASE HELP I want to use Medium.
