If your headset is still under warranty, get it replaced. I don't think an update or a factory reset will fix the problem.
You can contact support via email, chat or WhatsApp HERE
If it's nearly new—for example, a Christmas present—return it to where it was bought and get a new one.
If you're out of warranty, you can use a repair service like "Fix my Oculus" in the US. If you're in another country, just google for a repair service in your country.

Hey, thanks. I talked to customer support and they initiated a replacement for the headset.
