Quest 3 Controllers disconnecting at certain distance

I've been noticing lately that my hands will just suddenly become stiff if I stretch, grab something in VRChat or when I'm trying to  grab something on a current model  I'm wearing.

Its becoming increasingly worrisome that I can't even grab things on certain models that you can actively grabbed

Now I'm in a well lit room, my radius is lined by my room so plenty of space.  I just don't know what it is, please someone help.
