Same has been happening to me for months have hard reset even factory reset and it still happens.

I had one luck where I tried casting whilst my headset was off, obviously didn't connect, but when I turned my headset on I got a notification saying Quest is ready to cast and the menu appeared. Dunno if it was just luck as haven't had to test again but that might be a temporary solution.
Bit of a pain hey! The menu does come back but I have to literally turn the headset on and am off on average about 7 times before I get a menu come up!
when the menu does not load I’ll press my menu button and the new UI will show but nothing functions if you click it.
omits literally on off until the first main horizon menu comes back
