Entitlement check failed

All of this morning I'm getting "Entitlement check failed" intermittently and in the MQDH I'm seeing "not published" and published turning on and off randomly? 
Any idea how and why this is occuring?

    Actually ignore my statement about published or unpublished. I've coincidently decided that had something to do with my issue, but perhaps has no impact on it. If I build our app, same process, same headset, same account - I still see a fail for the entitlement check. 

    I've looked at the channel invites and my account is still listed as a user for each channel

    I'm logged in as me, went to the app PDP. Does this dropdown have any bearing on the entitlement?
