Maybe you should give some details about your rendering loop. Not just setting up.
If I remember correctly I also had a Rift LED staying orange when I had a problem with my ovrHmd_BeginFrameTiming() / ovrHmd_EndFrameTiming() pair. (But I am on DirectX, so it may behave differently.)
