Try restarting your headset, with this is still happening then factory reset the headset. (Turn on cloud data to save your progress.) With this is happening again then get in contact with Meta and try again to get in contact with them.

I don't have an answer for your question unfortunately, but do you develop in Unity? If so, how have you been able to get around the Unity automatically turning on banned permissions?
... Been having a lot of trouble with submitting apps to Meta and unfortunately get the feeling there is something shady going on with their treatment of indie developers... Intention preventing indie games from being released?

I personally develop on unity. I haven't had any issue with that. the only "Banned" permission i have on mine is the "android.permission.RECORD_AUDIO" which is used by photon voice for voice chat. If you game has one that seems like something you've added it is automatically turned on to fit the requirements of that add-on. So if there is something else it must be an error in your project.
