Developing for both PC and Mobile, but the problem only occurs on Rift. Upgrading to Unity 2017 and OVR 1.21 did fix the issue. The cubemap is working fine on GearVR Unity 5.6 with OVR 1.14. Tested OVR 1.18 through 1.21 and could not get any Overlay rendering to work in Unity 5.6.
We had already submitted a release candidate to the store when this issue began appearing so the Unity 2017 upgrade was a last resort! But for now this looks stable enough to release.
