3-Slide presentation of my game

Hello!

While reading this article at the very end, I came across a paragraph that says about a 3-slide presentation of the game for the oculus.

I can't find intake form in the support section.

Please tell me, have you experienced something similar? Do I need to send this presentation now? What should be in these slides and are there examples? Where and to whom should it be sent?

I can’t find answers in the documentation from Oculus, I will be very grateful for the information.
