App shows broken image on loading

I have noticed that some apps, when loading in Rift, do not display a main icon image, but instead show a grey broken image icon. Other apps, Star Trek for instance , shows an large icon of the game and a small icon in lower left frame. Where are these images stored and can you place your own images to load when an app hasn't provided any?
