Ok I have just purchased two headsets and unsure which way to set up. We want to play together sometimes. We also do not have a big house.. Is it better to have separate meta accounts or just one and add one person as extra player?

both of you need a Meta account if you want to play together.
Even if you don't play together is it better each of you have his own account.
That's the only way that you both can have your own progress and and save game.
If you set up both headset with the same account as admin and add the second account, you will be able to share games and don't need to purchase them twice.

Turn on App Sharing on Meta Quest headsets
Note: App sharing is nice for games that you play alone, but most multiplayer games requires that each account has its own copy of the game.....so, games that you want to play together need to be purchased twice.

Yes, if you you want to play together/against in a game - for example beat saber - you both have to buy it.
Multiplayer mostly does not work with a shared game.
