I have got my RMA after last blackscreen and lost tracking. I have been playing with it with extra careful. I got it days ago. Yesterday the headset reported lost headset tracking while i was not doing any training. I was using lay down mode for watching video. It turned blackscreen at last and not responsive. I tried to be creative and use my phone for casting the screen. Suprisingly the headset turned back on... But when stop casting it turned black screen again. So if I keep casting it works as expected. If i turned of casting it is black screen again. 

I am out of idea what went wrong....... When casting and playing, the headset sometimes lost tracking as well. One time it completely crashed.

Turn off the Proximity Sensor
The proximity sensor ensures the headset goes to sleep when not in use.
For development and debugging purposes, you can turn off the proximity sensor to keep the device in an active state. This can make it much easier for you to capture screenshots, record videos, and step through code.
To turn off the Proximity Sensor:
  1. In MQDH, select Device Manager from the left navigation pane.
  2. Under Device Actions, turn off Proximity Sensor to keep the headset in the active state for a select number of hours.
    You can also press the CTRL + Shift + P keyboard shortcut to turn on or off the proximity sensor.
Note: Turning off the proximity sensor means your device will not go to sleep, and your battery won’t last as long as it would with the proximity sensor on. When you no longer need the headset to be in an active state continuously, we highly recommend turning the headset off, leaving the device charging, or turning the proximity sensor back on.

i think i am the first one who found this workaround.. so if you have a black screen, try casting if it birngs back the headset. this must be checked and investigated by meta, whether this is hardware or software!.

reload, clear boundary nothing help. i have to keep casting while using. as soon as i stop casting, it gives black screen

@Kong777  having to cast to stop the screen from going black could be a sign that the proximity sensor is dying.

try disabling the proximity sensor with the meta developer hub utility and see if the screen stays on. If it does, it could be moisture damage to the sensor connection.

Thanks for sharing your thoughts on this issue. I agree with you it seems like the sensor is dying. Nice to know there is an option to disable it.

I think I read you post regarding the moisture damage. This quest 3 I am having is a RMA, I received it days (approximate 1 week) ago. I played only one fitness game with extra caution (sweat headbands, wipe the sweat and a facial interface which has holes for getting rid of steam.) Yes, I have never play this carefully before... 

After playing there was no issue the other day as well. I happened days after (no sweat no fitness game).

Talked to Meta support, I dont want to destroy the possibility of the casting workaround, no factory reset.. I perform a reboot from the boot up manual, it looks like fixed. I will try a few more days and see... My experience tells me factory reset never works.

By the way, where do I find the option on developer hub utility? I don't see that option?!

@Kong777  one other thought is, that you may have not got moisture in your latest headset, but if it's a reburb unit, then the previous owner may have only played thrill of the fight, and so the damage may already have been done by the time you got it.

@Kong777  extreme example of moisture damage, and the proximity sensor is also affected.
