Looks like the Note 4 was rooted/flashed before but would be strange coming directly from Samsung.
You could try installing the update via Kies software for the PC, as over the air doesn't seem to work.
If that also doesnt work, i would contact Samsung again and ask them how to install the update or get another Note 4 which already has the update.

The message "To open this application,insert your device into your Gear VR" is normal behavior for apps not downloaded from the Oculus Store. You cannot test your apps from inside the Oculus app, you need to launch the app manually.
So when you see that message, just put the phone in the VR headset and the app will launch automatically.
