Here is the solution to the three-dots (for me). This was posted on
Reddit and immediately fixed the problem for me. Please note, you need
to install the x64 version. I originally installed the x86 version, but
it doesn't include the required .dll So...
Since we're all just sitting here waiting for Oculus to eventually,
maybe, get around to getting our headsets working again, does anyone

Unfortunately v55 via PTC doesn't seem to be a universal fix. I opted
