My brothers rift just died like that.  But it went black in the middle of a game while we were playing co op.  After testing a lot of things, it turned out to be his cable, I gave him the cable from my headset to test with and he stress tested it for 2 days and confirmed it was 100% his cable that failed.  The clip on the left side of the HMD that holds the wire in place was not on the rift he received.  The cord twisted over time and had developed a short inside.  With any luck that's your issue and requires a $50 replacement cable that Oculus sells in their online store.  If it's like my issue was it may require an RMA if you are still in warranty.  Mine would make the computer play the USB sound and a pop up would say "HDMI Disconnect".  Then it would reconnect.  It would get worse the longer the HMD was in use.  So as it would warm up it would get worse.  If you so much as brush the side of it bam HDMI disconnect.  Then when you'd barely move BAM Hdmi disconnect.

So if the Disconnect is accompanied by a request box on windows, it's probably a solder joint on the HMD where the cord connects and will require an RMA.  If it just isn't showing any video, it's likely to be a simple cable replacement.  Do you know anyone else that lives nearby with a rift so you can test their cable on your headset?

Check the cable connection on both sides. Especially on the headset. You'll have to remove the facial interface bracket to see the plug. Also, USB issues can also cause that error. Please try different USB ports for the headset (both USB 3.0 and 2.0 are supported).
