Warranty replacement of a right quest 2 controller

About a week ago i figured out how to put in a warrantly claim for my right quest 2 controller where the trigger broke.  I put in the serial number for the controller and it said it is eligible for replacement via warranty.  It gave me a button to print a return label, but i was on my phone and couldn't print at the moment.  Now i try and recover that label and it is gone an cannot find where to reprint anywhere.  I tried to start over and it is telling me that it is now not eligible for replacement.  I need to speak to someone to have them send me the original return label.
