Unfortuneatly there are 3 possible places for disturbing smudges and dust in your view and that is the lenses facing your eyes, the Note 4 display and the back of the lenses facing the Note 4. I clean the display of the phone everytime i put it in Gear VR, then once or twice a day the lenses facing my eyes and once in a few days also the back of the lenses.
I have ordered watch protectors in 36mm seize for the lenses, to see if they will help with Fog, smudges and maybe even improve the screendooreffect.
