Have same problem. It happened after update to V60. Never had it before on V59. Now I have almost always after close some games like beatsaber. Need to restart device to fix it.

Hi there, 007x007! Making sure you're able to enjoy your VR experience to the fullest is definitely a priority for us, and we certainly want to help get that Passthrough working as it should! If you haven't already, have you been able to confirm if the passthrough comes back on when you double tap the side of your headset? In case you need it, here's the steps to make sure that is turned on:
- Press the logo button on your right Touch controller to pull up your universal menu.
- Hover over the clock on the left side of the universal menu. When Quick Settings appears, select it to open the Quick Settings panel.
- Select Settings in the top right corner.
- Select Physical Space.
- Select Passthrough from the left menu.
- Select the toggle next to Double Tap for Passthrough to turn this feature on.
- Hard reboot your headset:
- Turn off your headset.
- Firmly hold the power and volume down (-) buttons on your headset simultaneously for about a minute, or until the boot screen loads on your headset.
- Use the volume buttons on your headset to highlight 'Boot device' and press the power button to complete that.

I do not make hard reboot because have too many things to reinstall back and some apps can lose settings. Rest steps do not help.

Thanks for the swift reply, 007x007! We appreciate you trying out those steps for us, and working with us through this so far. To clarify, the hard reboot is not a factory reset, and will not uninstall or delete app data when performed. This is a simple reboot through the boot menu to help completely refresh that device, and should not effect your apps at all. When you have the time, please go ahead and try out that hard reboot, and let us know if it has any effect on the issue. In addition to that, are you able to let us know if double tapping the side of your headset turns on Passthrough? Do you have Passthrough turned on in the settings we sent earlier?

Ahh, hard reboot I did of course and it didn't help. Yesterday I got this. Look at menu edges. All around is black while should be passthrough. It happens only when I close any game/app. Not always, but in 30%-40% cases.
a) I have turned passthrough always. It just "jam" on black screen sometimes.
b) double tapping works and switch between passthrough and vr environment without any problems. Black screen happens only after close any game/app.
ps. OVR not cause this. I just installed it 2 days ago and problem with black screen was before 🙂
