Question

Input forwarding icons (mouse/keyboard) disappeared from Cast toolbar

**Setup:**
- Quest 3, tethered via USB-C to a Mac Mini
- Developing with Meta Spatial SDK (meta-spatial-sdk version 0.11.1)
- Horizon OS v2.6 (current as of this posting)
- MQDH 6.3.0

**Original issue:**
Input forwarding worked initially — I could see the mouse and keyboard icons in the Cast toolbar, enable the mouse icon, and get WASD movement working in my app (after fixing an unrelated BuildConfig.DEBUG bug in my own code). However, the mouse cursor itself never appeared in-headset (just a red dot, the default gaze cursor) and clicks/mouse-look never worked, even with WASD functioning.

**What I tried for the mouse-specific issue:**
- Confirmed Cast resolution set to Original (1:1)
- Confirmed only the mouse/headset icon was active (not the keyboard icon, which I understand disables WASD movement per the docs)
- Tested clicking and Ctrl+drag-to-rotate — neither worked
- Suspected macOS permission gating (Accessibility vs Input Monitoring being separate on macOS)

**What broke things further:**
After adding MQDH to both System Settings → Privacy & Security → Accessibility and → Input Monitoring, the mouse and keyboard icons disappeared from the Cast toolbar entirely (previously they were visible, just non-functional for mouse specifically).

**Troubleshooting done since (icons still missing):**
- Fully quit and relaunched MQDH (not just closed the window)
- Restarted the Mac Mini completely
- Quit Mac Mouse Fix (a third-party mouse remapping tool) in case it was intercepting the CGEventTap MQDH needs — no change
- Restarted adb server (`adb kill-server` / `adb start-server`) — headset shows as `device`, not `unauthorized`/`offline`
- Unplugged/reconnected the USB-C cable
- Toggled Accessibility and Input Monitoring permissions off and back on for MQDH, with a full quit/relaunch between toggles
- Fully uninstalled and reinstalled MQDH
- Re-added MQDH to both permission lists after reinstall

None of the above brought the icons back. The Cast window still shows video from the headset fine — it's specifically the mouse/keyboard input forwarding icons in the toolbar that are gone.

**Question:**
Has anyone seen this specific regression — input forwarding icons disappearing after a macOS permissions change — and found a fix? Also open to confirming whether this is a known bug worth filing separately. Happy to provide logs if useful.

Thanks for any help.
